Client received income on IRP5 with 4005 medical aid contribution of 17012. There is no 3810 or 4474 on IRP5, therefore I assume employee make the contribution and nothing from employer. Medical aid certificate says contributions of R56 712.00 - Do I use

One would use the amount contributed to the medical scheme as reflected on the certificate received from the medical scheme.  This amount will also be shown on the return of income (the ITR12).  

But we suggest that you confirm with the client as well.  It may well mean that the individual didn’t inform the employer that the contributions are made personally.
